WALMART PHARMACY
We use Walmart for all prescriptions and shots. All prescriptions must be sent to the pharmacy after the screening, and before the member is dropped off at any Famond Care Network Facilities. Case management must make sure they update the member's pharmacy on file before the member is admitted to Famond Care Network.
Our members need proper care and that starts by making sure they get all their prescriptions on time and with the right pharmacy.

Famond Care Network (FCN) serves male and female residents ages 18 and above who meet our admission criteria. Our residents are those with:
Substance abuse issues
General mental health needs
Serious mental illness
Co-occurring disorders
Resident may be self-referred, agency-referred, or court-ordered
FCN staffs collaborate to provide comprehensive care management services and are responsible for coordinating primary and behavioral health care and social services to address the whole-person needs. Health-home-specific goals are incorporated into the individual's care plan, including goals and time frames, community networks and supports, and optimal clinical outcomes. Our team identifies and prioritizes population-wide needs and trends so that appropriate treatment guidelines and interventions can be implemented.

CRITERIA FOR SERVICES
Be aged 18 and over
Have documentation of a recent physical examination and evidence of freedom of Tuberculosis
Be willing to participate in the behavioral health programs
Be currently sober
Complete an assessment and an initial treatment plan
Sign and complete intake documents including authorization forms
Have an issue that is appropriate for a residential behavioral health facility such as
- An impairment in functioning as a result of a substance abuse problem and/or mental health diagnosis
IMPORTANT POINTS TO NOTE ON TREATMENT CRITERIA & MODALITIES
1. SMI Designation: All our members are already diagnosed with SMI before being referred to Famond Care Network. The members already have an existing clinical team, with a psych doctor, case worker, peer support and other resources before coming to BHRF. If a member comes without an SMI designation, and based on our assessment, it is determined that they need an SMI evaluation, we will work with the team and get the right resources. In this case, they will be referred to Crisis Preparation and Recovery, Inc for SMI Evaluation.

2. Clinical Oversight:
Clinical oversight means oversight provided by an individual who, by virtue of education, train- ing or experience, is capable of assessing the psychosocial history of a substance abuser to determine the treatment plan most appropriate for the client. The person providing oversight shall be designated by the applicant.
This is done every 3 months or as needed after the initial intake process. This is done after observing the member, going through the daily progress records, monthly case staffing. The member’s case is staffed in the presence of the member and the staff. All the information gathered is used to determine the member’s progress to be measured on the treatment plan.
3. How do you determine when a member needs special services: Through initial assessment, member's history, current observations, behavior patterns, daily and monthly staff record reports. We then communicate with the case manager and everyone on the care team to figure out the best resources the member needs.
4. What will you do if a Tribal Member needs services: We will ask the member where they are coming from and locate the tribal behavioral Health authority to gather information and resources best available to work with the Tribal member. We will also follow these guidelines once everything is cleared with the tribal behavioral Health authority:
NB AIHP MEMBERS
Notification of admission to AHCCCS is required for initial coverage of up to 5 days of care. During this period, the BHRF is responsible for submitting a Prior Authorization request and ensuring compliance with the criteria listed in AMPM 320-V. If the Prior Authorization request and supporting documents are not received within the initial 5 days of admission, subsequent claims might be denied.
Admission documentation needed for authorization beyond the initial 5 days (all criteria for admission still must be met from the admission date):
A. Evaluation to determine Behavioral Health Condition and Diagnosis. Evaluation should be recent, and not older than 1 year. Done by a BHP, or by BHT and cosigned by a BHP, utilizing standardized instrument that is able to determine the appropriate level of care.
B. Treatment Plan - done by the Inpatient/Outpatient or TRBHA Treatment Team. Included in the plan should be an intervention specifying the BHRF level of care as necessary for the member as a least restrictive level of care required to treat the Behavioral Health Condition, identified in the Evaluation. This plan shall not be older than 3 months from the request submission date.
5. How often do you do individual therapy: two times a month in person (45 minutes to 1 hour). We touch on psycho-social aspects of their treatment plan.
6. How often do you do BHT group and one on one counseling: The groups and one on one are done one to two times daily depending on how busy the day is due to member’s doctor appointments and other activities that may hinder the smooth running of the groups. The times may also vary. These sessions are done with materials supervised by the BHP.
1. How often is clinical supervision and how: It is done twice a month both virtual and in person. Our online clinical supervision services are conveniently provided online via HIPAA compliant live video conferencing, at our Florence office, or at the supervisee's office.
