VERY CRITICAL MEDICATION HELP TO TAKE NOTE OF

STANDARD MEDICATION  TIMES AND ABBREVIATION 


1.   Twice (BID, 2 times daily) 8am,  8pm.


2.   Daily (once daily, 1 time a day QD) 8am.


3.   Three times daily (TID, 3 times a day) 8am 2pm 8pm.


4.    Every 8 hours 8am 4pm  midnight(12am).


5.    Every 12 hours   8am 8pm.


6.    4 Times a day (QID) 6am   12pm(noon) 6pm 12am(midnight).


7.    As Needed (PRN) - No scheduled times (for example if order reads every 4-6 hours 8hrs, or 12hrs as needed, if client request meds at 8am, the next time they can have the PRN will be  4 or more than 4hrs from 8am (12noon and above) or 4pm or above for every 8 hrs. as needed).

MEDICATIONS

1. Check medications if 7 days or less with refills,

2. Call Saliba's Pharmacy (623) 815-8965 Or the pharmacy on file.

3.  Leave a note for the next staff.

4.  If a med is taken 2 or 3 times a day, then 14 or 21 pills left need a follow-up.

5. Complete a new Mar/Med log, update the med list when a new medication is prescribed and delivered.

6. Check for refills in the refill box 7 days before a med runs out and when the bottle is out to make sure there are no pills in the bottle and refill section before signing out.

7. Sign off on mar/med logs every time you pass out meds. Never leave blank. Fill out the client's section with the codes found at the bottom of the Mar. Staff must initial and date when the client takes the med or not.

8. All controlled meds must be recorded, counted, and accounted for. No mistake is acceptable. Every new order of controlled meds must be started on a new controlled med sheet. 

CONTROLLED MEDICATIONS


Lorazepam, ACETAMINOPHEN-CODEINE, Vicodin, Oxycodone, Adderall, Xanax, Ambien, Lyrica, and  Percocet.
